Coverage is provided through Blue Cross PPO. The plan is a High Deductible Plan. The employee is responsible for $1,250 (single) and $2,500 (family) while the employer funds $1,750 (single) and $3,500 (family) to meet remainder of the deductible. The plan offers a telemedicine program which is a convenient and low-cost option when unable to get to a doctor.
Our dental coverage is provided by Aetna Dental. We offer both a PPO Plan, where you can use a network or a non-network dentist, and a DMO Plan, where you must select and use a primary care dentist. Our dental programs give freedom of choice which allows flexibility to move between the two dental plans throughout the plan year.
The company pays for a life insurance policy which is 2 times your Base Annual Earnings to a maximum of $250,000.
This is a company paid benefit. After 7 days of disability for either sickness or accident, the benefit kicks in for up to 12 weeks. The benefit pays 60% of Base Annual Earnings to a maximum of $750 per week.
Our PTO plan is competitive and designed to allow employees time off to recharge and relax. PTO is accrued on a bi-weekly basis and credited to your PTO bank in the timekeeping system.
This is a company paid benefit. After 13 weeks of short term disability, the benefit kicks in at up to 60% of Base Annual Earning to maximum of $6,000 per month.
We offer 10 paid holidays per year. The schedule is posted in January of each year noting the holidays for the upcoming year.

Innovative Benefit Planning provides employees with a single point of contact for help with anything benefits related. Your Benefit Guardian will assist you with any claims/billing issues you may have and with navigating the healthcare system. You are kept fully informed during any inquiry process.
Your Benefit Guardian from Innovative will also assist you in selecting or changing your primary care provider, as well as finding specialists and hospitals in your network. You may also contact Your Benefit Guardian to order replacement membership ID cards or to help with any benefit plan questions. Supplemental Life Insurance:
Additional Life Insurance is available for you to purchase on yourself, your spouse, and your child(ren) on a voluntary basis. Premiums are based on your age and the coverage amount selected (spousal rates are based on you age.)
Accident Insurance:
Accident Insurance covers you on and off the job and pays in addition to all other insurances. Benefits are paid for specific expenses when an accident occurs.
Hospital Indemnity Insurance:
Hospital Indemnity Insurance pays on top of all other insurances and is designed to help offset deductibles & copays when hospitalized. This benefit is paid when hospitalized due to a sickness, accident or injury.
Critical Illness Insurance:
Critical Illness Insurance pays on top of all other insurances. A lump sum benefit when diagnosed with certain critical illnesses. The plan also includes an annual wellness benefit, which pays $50, per insured, per year up to a max of three payouts per year, per policy. Examples of wellness tests covered are mammograms, colonoscopies, Pap smears, stress test on a bicycle or treadmill, fasting blood glucose test, PSA, etc.
The 401K plan is held by Fidelity. There is Quarterly Open Enrollment. The plan offers many Investment Options including Target Date Retirement Funds. Title Alliance matches 25 cents on the dollar up to 6 percent of your annual salary.
