The company pays for a life insurance policy which is 2 times your Base Annual Earnings to a maximum of $250,000.

Long Term Disability

This is a company paid benefit. After 13 weeks of short term disability, the benefit kicks in at up to 60% of Base Annual Earning to maximum of $6,000 per month.

Medical

Coverage is provided through Blue Cross PPO. The plan is a High Deductible Plan. A Single Deductible is $3,000 and a Family Deductible is $6,000. There are no referrals needed and the plan pays 100% after deductible except for prescription co-pays which are $20, $40 or $60. The Company offers a Heath Reimbursement Arrangement (HRA) to assist with the deductible. The HRA is Company funded. It funds $1,250 (single) and $2,500 (family). Employees are responsible for the next $750 (single) and $1,500 (family) to meet the deductibles.
